You will receive an email confirming your screening time. If you do not have access to the internet you may call our school office at 408-874-3600 to set up a screening time.

You may also complete the Kindergarten Questionnaire online.  Also, we like to be able to match students’ names with their faces on the first day of school.  For this reason, we ask that you attach a recent photo of your child to the questionnaire that we can keep. (If you complete the questionnaire online just bring your student’s picture with you the day of the screening.)  This photo will not be returned to you.  If you are unable to bring a photo, we will take a picture of your child during the group screening.
